The information available can often be confusing, particularly when medication is discussed without the medical context in which it is prescribed. Understanding where misoprostol fits into a medical abortion, and why professional assessment is still essential, can help you make a safer and more informed decision.
Medical abortion uses two prescription medications that are taken in sequence to end an early pregnancy. In Singapore, these medications are regulated under the Termination of Pregnancy Act and can only be prescribed by a licensed abortion provider following an appropriate medical assessment.
The first medication prepares the body for the abortion by stopping the pregnancy from continuing. Misoprostol is then prescribed as the second medication to complete the process. Using both medications in the correct order improves the effectiveness of treatment.
Misoprostol is usually taken 24 to 48 hours after the first medication, following your doctor’s instructions. It works by causing the muscles of the uterus to contract, allowing the pregnancy tissue to be expelled. As this happens, most women experience cramping and vaginal bleeding that is often heavier than a normal menstrual period. Nausea, diarrhoea, chills, a temporary fever and fatigue may also occur.
The timing and intensity of these symptoms vary. Some women begin bleeding within a few hours, while others may take longer. The heaviest bleeding usually occurs shortly after taking misoprostol, followed by lighter bleeding or spotting that can continue for one to two weeks.
Even after the bleeding has settled, medical abortion is not considered complete until your doctor confirms that the pregnancy has ended successfully. A follow-up appointment may include an ultrasound or other assessment to ensure that no pregnancy tissue remains and that the pregnancy has not continued. This is important because complications such as incomplete abortion or an ongoing pregnancy may not always be obvious based on symptoms alone.
Your doctor will also review your recovery, answer any concerns and discuss contraception if appropriate, as fertility can return before your next menstrual period.
Medical abortion may appeal to women who prefer to avoid surgery. However, it is important to recognise that the process takes place over several hours or longer and relies on the body responding to medication as expected.
For pregnancies beyond the early weeks, or where medical abortion is unsuitable, surgical abortion may be recommended instead. The procedure is completed within a short period under medical supervision, providing a more controlled treatment with a lower likelihood of incomplete abortion requiring further intervention.
